چکیده

We read with interest the excellent retrospective study of YooSeok Kwon, reporting a high proportion (22.9%) of patients with transient global amnesia (TGA) and epileptiform discharges (mostly with a left dominance) detected within days of the episode of TGA. Authors should be commended for having conducted a study aimed to shed further light on the pathogenesis of such a fascinating disorder. It is noteworthy to consider that epileptiform discharges on the EEG are not by themselves synonymous with epilepsy. Previous reports in the literature described in EEG recordings of these patients a frequent occurrence of Subclinical Rhythmic Electrographic Discharges of Adults (SREDA), an otherwise uncommon distinctive rhythmic pattern seen on EEG in subjects older than 50 years, considered to be unrelated to epilepsy. Surprisingly, this rare EEG pattern, which has a prevalence of 1 per 2500 recording, has been reported to occur much more frequently (3–25%) among TGA patients. The correlation of SREDA with mechanisms which produce TGA is debatable, although – overall considered – it seems unlikely. However, the risk of considering this spiky pattern fortuitously found in EEG of patients with TGA as truly epileptic, with following misdiagnosis of transient epileptic amnesia should be taken into account and avoided. This and other EEG patterns of uncertain significance that occur without any correlation with epilepsy should be well known by neurologists and promptly recognized to avoid misdiagnosis of epilepsy due to an overinterpretation of normal sharp patterns.

Subclinical rhythmic electrographic discharges of adults and transient global amnesia: a causal or casual association?

Subclinical rhythmic electrographic discharges of adults (SREDA) is an uncommon distinctive EEG rhythmic pattern with uncertain significance. We report a patient with transient global amnesia in whom an EEG recording, performed after the acute phase of the neurological event, showed SREDA. Based on this EEG finding, we discuss about a possible correlation between SREDA and transient global amne...

Transient global amnesia mimics: Transient epileptic amnesia

We describe the case of a 79-year-old patient referred for suspected transient global amnesia, after an episode of anterograde amnesia which lasted 90 min. An EEG, performed after the episode, showed bilateral temporal electrographic seizures, orienting the diagnosis toward a transient epileptic amnesia. Transient epileptic amnesia is defined by temporal lobe epilepsy characterized by recurrent...

Left dominance of EEG abnormalities in patients with transient global amnesia

PURPOSE Transient global amnesia (TGA) is a syndrome of unknown etiology. Electroencephalographic (EEG) abnormalities in TGA have been reported previously. We analyzed the frequency and characteristics of EEG abnormalities in patients with TGA. METHODS We collected EEGs of patients with a clinical diagnosis of TGA who had visited the emergency room or the outpatient clinic over a period of 8 ...

Transient global amnesia with post-hyperventilation temporal sharp waves—A case report

We report a 55-year-old woman, who presented with transient amnesia for 7h following underwater swimming. There was no evidence of neurological disturbance except global amnesia. Sharp waves in both temporal regions were registered during the initial EEG recording, which was accentuated by hyperventilation. Right hippocampal high signal intensity was observed in DWI performed 56 h after symptom...

Transient epileptic amnesia

Transient epileptic amnesia has been considered a syndrome of mesial temporal lobe epilepsy characterized by (1) recurrent episodes of isolated memory impairment of epileptic cause (ictal or postictal) while other cognitive functions remain intact; (2) interictal memory disturbances of accelerated long-term forgetting and autobiographical and topographical amnesia; and (3) late age of onset wit...
